EVOLUTION SPORTS DATA PROTECTION AND CONFIDENTIALITY POLICY
 
EVOLUTION SPORTS will collect and use/process your personal information in the manner described below.
 
A) INFORMATION COLLECTION - NOTICE AND DISCLOSURE STATEMENT
 
I. EVOLUTION SPORTS  may utilize your personal information for the following purposes:
1. Member Verifications
2. Statistics collection
 
B) CONSENT/CHOICE STATEMENT
EVOLUTION SPORTS  does not sell or rent personal information collected from you to third parties.
 
You explicitly specifically consent to EVOLUTION SPORTS  and its authorized representatives to use the data for the purposes mentioned in paragraph I above.
 
You acknowledge that your personal information may be transmitted and used/processed by EVOLUTION SPORTS  and its authorized representatives entities outside of your home country/jurisdiction and explicitly specifically consent that your personal information can be transmitted stored and processed in such third countries for the purposes specified in paragraph I above.
 
C) DATA QUALITY AND SECURITY STATEMENT
 
EVOLUTION SPORTS uses security measures in order to protect the data we have under our control against manipulation, loss, destruction, access by unauthorized persons, misuse, alteration or against unauthorized disclosure. You take on the personal responsibility of ensuring that your password is kept confidential. EVOLUTION SPORTS will not be liable for the manipulation, loss, destruction, unauthorized access, misuse, alteration, or unauthorized disclosure of your personal information due to negligence or failure on your part to ensure the confidentiality of your password.
 
Our security procedures are continually enhanced as new technology becomes available.
 
EVOLUTION SPORTS does not sell or rent personal information collected from you to any third parties.
 
We would like to use your data for the purposes specified above. Naturally your participation in such collection of personal information for the specified purpose is voluntary. Should you not consent, you can inform us at any time, so that we can prohibit the use of your data accordingly.

Release of Liability :

EVOLUTION SPORTS RELEASE OF LIABILITY
 
I, being the parent/guardian of the aforementioned student(s).
 
I acknowledge that the above information is factually correct. I therefore agree to them participating in the activities of the EVOLUTION SPORTS held on all locations, and the use of all our sports equipment.
 
I agree that I will not hold the Coaches of the Academy or the site liable for any losses, liabilities and damages, injury or death suffered or incurred by my child whilst taking part in any of the activities run by EVOLUTION SPORTS.
 
EVOLUTION SPORTS MEDIA CONSENT
 
* Photographs and / or videos may be taken of my child taking part in sporting activities for promotion of the club and displayed on EVOLUTION SPORTS website, social media, press releases, articles, and in newsletters in accordance with child safeguarding guidelines.

Refund Policy :

EVOLUTION SPORTS REFUND POLICIES
 
Fees are non-refundable unless it is due to extenuating circumstances that is medically related. Each case will be assessed on its merit with the provision of the necessary documentation.
    
An administration fee of QAR 250 will be applicable. 
    
Administration fee of 450 QAR is applicable to refunds other than medical grounds.
    
Refunds will be processed within 3 weeks from the date of approval. 
    
We reserve the right to deliver our programmes online if any government restrictions relating to the COVID-19 pandemic prevent the physical delivery of programmes. No fees will be returned in such instances.

EVOLUTION SPORTS COVID-19 DECLARATION
 
Dear Parent,

Under the guidance from the Ministry of Education, we will be reopening EVOLUTION SPORTS with a number of precautionary measures in place.  We are working on the assumption all members will have completed the quarantine requirements, if travelled internationally, as per Qatari law by the time the child returns to training.

Each morning, before training with EVOLUTION SPORTS we need you to complete the following steps with your child/children.

* Make sure your child does not have a sore throat or other signs of illness, like a cough, shortness of breath, difficulty breathing, fatigue, muscle or body aches, congestion or runny nose, diarrhoea, severe headache or vomiting.  If your child/children display any of these symptoms, they must stay at home and inform their child's coach. 

* Take your child/children's temperature, each morning before training. If the temperature is 37.8°C or higher, you must keep your child/children at home and inform their child's coach. 
    
* If your child has had close contact with a COVID-19 case, you must keep your child/children at home and inform their child's coach.

Once your child/children are in an Evo Sports facility, their temperature will be taken by the security team and coach.  If their temperature is above 37.8°C, then they will be isolated and you will be asked to come and collect your child immediately.

For students aged 12+ the coach will also check the Ehteraz app status. Any students who do not have a green status will be isolated and you will be asked to come and collect your child immediately.

We request that you work with us in ensuring the health and safety of all children and staff at EVOLUTION SPORTS and for this your cooperation is required.
